Launched by the Centers for Medicare & Medicaid Services (CMS) on July 1, 2024, the Guiding an Improved Dementia Experience (GUIDE) model is a groundbreaking, no-cost care initiative focused on individuals living with dementia and their caregivers. Running through 2032, this nationwide model aims to transform dementia care for Medicare beneficiaries by providing long-term, condition-specific, and culturally inclusive support.
Why GUIDE Matters
Dementia affects more than 6.7 million Americans and is projected to more than double by 2060. Patients often face fragmented care, frequent hospital visits, and high emotional and physical stress. At the same time, unpaid caregivers – many of whom are Medicare beneficiaries themselves – struggle with the intense demands of care.

Who Can Participate?
To qualify for GUIDE, individuals must:
- Be enrolled in Traditional Medicare (Part A and Part B)
- Have a primary diagnosis of dementia
- Live at home, in an independent or assisted living, or a memory care community
- Not be currently enrolled in hospice or living in a long-term skilled nursing facility
